Care Pathways and Guidelines

Introduction

The pathways and guidelines are designed to support the services provided by healthcare professionals to patients across primary and secondary care in Haringey.

They have been developed with input from key professionals, are based on national clinical guidelines and will be reviewed and amended accordingly. Pathways and guidelines will only be included here if they have been appropriately approved within the TPCT e.g. the Professional Executive Committee (PEC) and /or the Clinical Effectiveness Group.

Benefits of using Care Pathways

  • To ensure the care and treatment provided to patients is based on the most current national guidance
  • To improve the quality and consistency of care for patients
  • To reduce variation in care and treatment
  • To increase communication across multidisciplinary teams and support effective team working
  • To assist the transfer of care between disciplines, thus improving the quality of care for patients
  • To identify and address gaps in the delivery of services
