Same-Sex Accommodation

Over recent years the Dept of Health has asked NHS providers for assurance that they follow guidance on mixed sex accommodation and each time most providers declare that they do follow this guidance, however the message from patients seems to differ.

Each year the Care Quality Commission conducts a survey of NHS inpatients, which includes questions about mixed sex accommodation. The link below shows an extract from 'Privacy and Dignity: The elimination of mixed sex accommodation: Good Practice Guidance and Self Assessment Checklist' and explains what people have a right to expect from the NHS with respect to mixed sex accommodation. NHS Haringey are compliant in achieving this standard.

NHS Haringey manages Greentrees which is a 50 bed Rehabilitation Unit for older people located on Beech and Cedar Wards at St Ann's Hospital. It comprises the 6 bed Acorn Stroke Unit, based on Beech ward delivering intensive rehabilitation to stroke patients. Cedar ward has 6 continuing care and respite care beds. The remaining 38 beds on both wards are used for older people who require a period of rehabilitation after admission to The North Middlesex University Hospital often following stroke or orthopaedic surgery.

Safeguarding adults

Lead Nurse

Martin Grant is the lead nurse for safeguarding adults
His role is to provide evidence based clinical leadership on all aspects of the Safeguarding Adults agenda to NHS Haringey and other health service providers.

His professional background is varied having worked primarily in forensic and liaison psychiatry, and psychotherapy. He also trained in law before specialising in medical and mental health law. In his previous role, he taught relevant law to medical, clinical and social care professionals, from mentally disordered offenders to consent/refusal of medical treatment.

Part of his post is to be responsible for the development and implementation of a plan for Safeguarding Adults that ensures safeguarding is in line with national policy and guidance; is consistent across Haringey; and co-ordinated with Haringey Council, the lead investigatory body.

Professionals are highly aware of their responsibilities around child safeguarding; responsibilities that have grown significantly since the Children Act 1989. Safeguarding adults remains the poorer sister to its younger yet vintage sibling. Many professionals are unaware of how and when to raise concerns around suspected neglect and abuse of vulnerable adults, or whether the adult services will investigate their concerns.

The investigation of concerns raised, begins with an alert to adult social services. An investigation is then held through a ‘strategy meeting’ where interested parties, including the person who raised the concern, discuss the risks and possible outcomes.  A plan is then made and acted upon. Historically, the multi-disciplinary team would assess and manage risks around safeguarding adults; adult social services now have a statutory duty to investigate and co-ordinate a response, with the police where necessary.

There are continuing changes around safeguarding adults that all professionals must be aware of and indeed act upon. From the new criminal act of wilful neglect and abuse of a vulnerable adults in the Mental Capacity Act; through to the new deprivation of liberty safeguards for compliant incapacitated adults, the safeguarding agenda is now central. The Care Quality Commission has now made safeguarding adults a core component in their inspections of NHS sites.

Chronic Pain service - Haringey residents only

The Back to Life Programme is a psychologically based rehabilitative programme for people with chronic pain which remains unresolved by currently available medical and other physically-based treatments. The aim of the programme is to reduce the disability and distresses associated with chronic pain as well as improve the coping strategies of sufferers by teaching physical, psychological and practical techniques to improve the quality of life.

Patients must:

  • Have experienced stable chronic low back pain for at least 6 months
  • Be between 18 and 65 years old
  • Have no symptoms of serious spinal pathology or inflammatory joint disease
  • Have completed all diagnostic procedures

Patients referred into the service are offered a face-to-face joint psychology / physiotherapy detailed assessment to ascertain whether the programme will fulfil their needs. The assessment process includes management options, recommendations and onward referral when the patient is not suitable or able to attend the programme.

Back to Life is held at Tottenham Green Leisure centre and runs for ten sessions, twice a week for consecutive weeks. Patients participate in group based discussion sessions, graded exercise and hydrotherapy. The course is led by experienced psychologists and physiotherapists in the field of pain management.

To refer into the service use the specific referral form that can be obtained directly from St Ann’s physiotherapy department on request, or by emailing a request to Jackie.Wilkinson@haringey.nhs.uk (Physiotherapy Clinical Specialist – Pain) and an electronic copy can be forwarded.

The referral has to be signed by the referrer and the patient, and all relevant sections completed.

Continence Advisory Service

A team of specialist nurses and support staff who provide a service to all individuals resident in Haringey who have an incontinence problem.

The Continence Nurse Specialists take direct referrals for active treatment of continence problems at the nurse-led Continence Clinics at Bounds Green Health Centre and the Urology Department North Middlesex Hospital.

Enuretic nurse-led clinics are held at Bounds Green and Lordship Lane health clinics for children who bed wet.

The Advisory Service team regularly provides training and updates for community nurses in continence assessment and gives advice on treatment and management (including products). They also offer support and training to carers and service users.
The home delivery service of continence products is managed from Bounds Green Health Centre by the service.

The district nurses, school nurses or health visitors carry out the majority of the assessments for management of bladder or bowel problems, but if the problem is complex, they refer to the nurse specialist.

District Nursing Service

The District Nursing Service of Haringey provides a range of skilled nursing interventions to house bound patients, that cannot be provided by either formal or informal carer’s. They provide this support to patients with identified health needs, and empower patients to maximise their independence . The teams provide an excellent range of care to people in their own homes including;

  • Palliative care
  • Daily administration of medications and insulin
  • Administration of gastrostomy feeds
  • Wound care
  • Urgent administration of analgesia
  • Administration of chemotherapy

Patients should be:

  • Resident within the London Borough of Haringey
  • Housebound
  • Have a clear need for nursing intervention

The Haringey District Nursing Service has an open referral system, which can be accessed directly via the Health Centres listed below or the GP surgery.

Common conditions or reasons for referring to the district nursing service include:

  • Care Management/contributory assessment of complex care packages
  • Long term management and support to patients with chronic degenerative conditions
  • Wound Care
  • Teaching self care procedures to enable patients to manage their own care needs
  • Terminal and palliative care
  • Preventing health complications associated with immobility, disease and disability
  • Advice and support for carers

Whenever possible, referrals should be made in writing using the District Nursing Service/SAP Referral forms and current medication. These can be obtained from the nearest health centre. The Haringey Foot Health Service provides care for Haringey residents identified as at risk of foot health complications. For example, people with:

  • Diabetes with complications, as described in the NICE guidelines http://www.nice.org.uk/pdf/CG010quickrefguide.pdf.
  • Rheumatoid arthritis
  • Severe hardening of the arteries in the legs and feet
  • Any illness needing long term steroids
  • Any illness that renders the foot liable to infection, ulceration or gangrene
  • Registered blind
  • The service offers a range of interventions for the above patients including education on foot care, the care of corns, calluses, thickened toenails, the treatment of ulcers and acute infections of the feet.
  • There is an emergency service available to deal acute problems of those patients acceptable for treatment.
  • The service is available at health centres or in patients' own homes if a podiatrist has assessed them as housebound.
  • The service does not provide simple nail cutting or treatment for verrucas.
  • Diabetic Clients
  • Screening the feet of patients with diabetes can identify those factors that contribute to the risk of foot problems, particularly callosities, nail deformity, peripheral vascular disease and neuropathy.

People with diabetes maybe referred to the service as per the NICE guidance:

  • When corns or calluses develop.
  • When there is a nail deformity.
  • When pedal pulses are absent.
  • Where there is peripheral neuropathy.
  • These are the factors that put the feet of patients with diabetes at risk of ulceration, infection and gangrene. Prophylactic chiropody/podiatry can significantly reduce the onset and reoccurrence of ulceration caused by increased pressure areas.
  • Foot Health Education helps patients with diabetes to take measures to prevent foot problems developing, to recognize them if they do, and to be aware of what action they should take if this should occur.

Foothealth Training

The Assessment of the Diabetic Foot.

We offer training to healthcare professionals e.g. GP’s, Practice Nurses in line with the NICE guidance. This enables these professionals to perform their own diabetic foot assessments. The training will teach:

  • Testing of the foot sensation using a 10g monofilament.
  • Palpation of the foot pulses
  • Inspection of any foot deformity and footwear.
  • Identification of foot health risk in accordance with the NICE guidance.

Greentrees

Medical leadership is provided by Medicine for the Elderly consultants from NMUH. The experienced multidisciplinary team provide a range of specialist services and therapies – Nursing, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Dietetics, Music Therapy, Clinical Psychology and admin. The wards are well equipped with excellent rehabilitation facilities including a kitchen for assessment and a gym.

Haringey Community Palliative Care Team

This service aims to provide and promote the principles of palliative care by being a resource for advice, support and education to people with advanced progressive disease. This includes cancer, HIV, motor neurone disease and advanced respiratory and cardiac disease. This support is provided to patients, their families, carers and to health professionals in the community involved in caring for this client group within their own homes.

This nurse-led service is made up of 6 Clinical Nurse Specialists, a visiting Consultant in Community Palliative Care and a Social Worker. The service is available 9-5 seven days a week.

This service is based at St Ann’s Hospital but covers all Haringey residents. Referrals can be made by patients, GPs, district nurses, hospitals and families.

Health Advisors for Older People

Four specialist nurses provide general assessment and health advice and information to older people and their carers in Haringey in order to support them to keep well. They work closely with other health and social services.

Integrated Cancer Care Programme

The Integrated Cancer Care Programme (ICCP) is a collaborative project running for eighteen months between the Department of Health and UnitedHealth Europe, with the main objective of improving the patient pathway for people with cancer through better coordination of care and individual case management. Haringey is one of nine pilot sites chosen across the UK for this programme. Haringey’s model utilises a telephone-based nurse (Care Coordinator) who has regular contact with the cancer patients enrolled in the programme. She answers questions, offers advice and support, and performs risk assessments to determine the need for physical review. Face-to-face contact is then carried out by the Community Matron (Cancer), or a member of the District Nursing Team. The pilot in Haringey is working with ten GP practices, five in the Bounds Green area, and five in Tottenham.

Intermediate Care Rehabilitation Team - New Name ICTT

Part of the Intermediate Care Services in Haringey, this team offers physiotherapy and occupational therapy rehabilitation to Haringey residents aged over 50 in order to avoid hospital admission or to support prompt discharge from hospital.
The team is based at Stuart Crescent Health Centre, with the Intermediate Care Social Work team, and can be contacted on 020 8889 1059.

North Middlesex University Hospital Therapies

The Integrated Therapy Service based at the North Middlesex University Hospital includes Physiotherapy, Occupational Therapy, Speech & Language Therapy, and Dietetics & Nutrition. These specialist services aim to provide prompt, effective and patient centred care, focussing on optimising the health status of individuals and facilitating safe discharge from hospital. The Dietetics, Speech & Language Therapy and Physiotherapy services also provide some outpatient services for clients referred by a consultant at the hospital, their GP or, where appropriate, to follow-up care on discharge from hospital.

Access to Specialist Mental Health

There are several key points to the improvements we have made at each step to achieve our aims.

  1. Step 1 - GPs and primary care staff are better equipped and able to identify and initiate appropriate care for people experiencing mental distress and emotional difficulties.
  2. Step 2 - we have increased the capacity of step 2 services to allow more people to receive evidenced based treatments in primary care and community settings.
  3. Step 3 - we are working together as a consortium so that the primary care mental health care pathway is simplified and access to the right service is sped up .
  4. Step 4 and 5 - the introduction of a single point of access into specialist mental health services with simple and clear care pathway including exit routes back into primary care.
