What is it?
The NHS Condition Management Programme (CMP) has been designed to:
- Improve your health and wellbeing
- Better understand your condition
- Regain control of your life
- Improve your self confidence
- Help you return to work now, or in the future
What does the Condition Management Programme offer?
It includes a range of short therapeutic interventions tailored to your needs lasting between 6-12 weeks. Subject areas include:
Core Topics:
- Managing your condition & work
- Pacing & Goal setting
- Healthy Lifestyle
- Confidence Building
- Positive Thinking
Support Topics:
- Relaxation
- Coping with stress/anxiety/panic
- Managing your pain
- Understanding and coping with low mood
- Coping with fatigue
- Sleep
- Healthy work plan
The topics are delivered in one to one and group sessions and you may be referred to other services if identified in your personal health action plan.
Who is it aimed at?
The Condition Management Programme is aimed at any Haringey resident who has a long term health condition and would like to return to work now or in the future.
Taking part in the Condition Management Programme is entirely voluntary. Whether or not you choose to take part will have no effect at all on your benefit.
Referrals
Referrals can be made by yourself or by an employment or personal advisor, health professional or G.P.
