Community/Intermediate Diabetes Specialist Care Service
About Our Service
Diabetes Specialist Care is led by Diabetes Specialist Nurse providing Intermediate care services for people with diabetes including management of complex patients on insulin and insulin initiation, development of care pathways and guidelines for the management of diabetes across Haringey. Encouraging self management through development of Handheld Record, Discovering Diabetes Information Booklet and structured patient education. Providing training and education for heath care professionals in Haringey to improve the care provided for people with diabetes.
Referrals to clinical service should be made using the SAP form and sent through the central referral point based at St Ann’s Hospital

We accept referrals meeting referral criteria from all health and social care professionals based in primary, secondary or community care.

Information for Health Professional and GP's
The Intermediate Diabetes Specialist Nurse-led Clinics
Who is eligible: Adults 18 years and over with Diabetes who are resident in the London borough of Haringey. Patients on 2 more or more maximum oral hypoglycaemic agents with HbA1c ≥ 7.5% or those already on insulin having problems (low blood glucose) or not reaching target. The service presently does not cover those already under the Hospital Diabetes Team or require home visits.
