Community Matrons

A service for Haringey residents to help them manage their long-term health conditions.

Who are the Community Matrons?

  • Community Matrons are highly skilled and experienced nurses, who specialise in the patient-centred management of long-term health conditions.
  • Being patient-centred means that they will ask the person about what is important to them and work with them to plan how to cope better.
  • Long-term conditions are health conditions such as heart disease, breathing problems, diabetes and Parkinson’s disease. People may have had these illnesses for a long time and find them stressful and difficult to live with
  • The community matrons work closely with GPs, hospital doctors and other local health and social care services to ensure that people work together to deliver planned care that supports people to be confident in managing their health.

Who is the service for?

  • Haringey residents with a long term health condition, who are likely to have unplanned A & E or hospital attendances.
  • People who have more than one active long-term condition, which is treated by their GP, and who would benefit from additional support to understand and manage their health.

What do they do?

  • Co-ordinate people’s care in the place they normally live.
  • Work with people who have a lot of hospital visits or admissions to help reduce unnecessary hospital attendance.
  • Develop a plan of care, to include measures the person could take to avoid a crisis
  • Support and inform, in order to improve people’s confidence in managing their health condition.
  • Act as a key contact for patients and their families
  • Liaise with other agencies and professionals to secure additional support as needed.

Getting in Touch

Community Matrons are based in the same locations as the District Nurses and work closely with them. Potential users should discuss referral to this service with their GP in the first instance.
